Digital Contact Hours Certificates


We will again be providing digital Contact Hours certificates this year.

Following the event, attendees will receive an email containing a link to a Google Form. This form will serve as your Conference Attendee Attestation, allowing you to confirm the sessions you attended. After submitting the form, your Contact Hours certificate will be automatically generated and sent to you by email.


*Please note MAOT’s new certificate policy*

In the event that you did not receive or have misplaced a certificate, attendees have up to 3 months after an MAOT event to email info@maot.org to request their event certificate without penalty. After the 3 month window, a $10 processing fee will be required to have your certificate re-issued.
